Chronic Disease Reviews

Specialist nurses and health care assistants and our team of clinical pharmacists can assess, review and support patients with diagnosed conditions including asthma, COPD, diabetes, heart disease, hypertension, kidney disease and dementia.

Daily telephone and face to face clinics (routine and urgent)

With a variety of health professionals including advanced nurse practitioners and doctors.

Melksham Older Peoples Team (MOPT)

This multidisciplinary team covers Giffords Surgery and Spa Medical Practices serving the needs of our senior population, including those in local nursing homes. The team includes specialist nurse practitioners and administrative staff with access to Alzheimer's support and social services liaison, bringing together the health and social needs of this growing population. The quality of care and support for this group of patients has improved significantly since implementing this service in 2016.

Mental Health

A specialist mental health practitioner whose expertise and advice is valued by both patients and colleagues is a fantastic addition to our multidisciplinary team.

Minor surgery and joint injection clinics

These are provided by doctors for a variety of conditions.

Multi-Agency Breastfeeding Support in Wiltshire

We know that breastfeeding rates in Wiltshire are below what we would like them to be, despite good initiation rates around 80% in Wiltshire, this drops to 48% at 6-8 weeks. The benefits of breastfeeding are well evidenced and we continue to promote these messages to our patient population. Breastfeeding Support in Wiltshire was developed by a multi-agency group of professionals who promote the benefits of breastfeeding and highlight the wide range of support available to women and families who would like to breastfeed.

Non NHS Services

There are certain services that are not covered by the National Health Service, for example, fitness to drive medicals and completion of private health insurance claim forms. When charges are made we use the British Medical Association scale of recommended fees. A copy of the fees is displayed in the waiting areas and on the practice website.

Other nurse led services

Cervical smears, INR monitoring, dressings and removals of sutures, child and adult immunisation, blood samples, ECGs and urine tests.

Sickness Certificates

Under current legislation a patient self-certificate is used for the first 7 days of an illness. The self-certificate (form SC2) is available from your employer, the Post Office or by visiting the HMRC website. Some employers insist on sickness certificates where the patient has been sick for less than 7 days. As this is not a statutory requirement, a charge may be made for issuing a certificate in these circumstances.

After seven days certificates may be obtained following consultation with a health professional or by telephoning the surgery and speaking to a health navigator.

Social Prescribing

People may visit their GP because they are feeling stressed about their work, money, or because they are lonely and isolated. This can impact on physical and mental wellbeing.

The Social Prescribing service can help.

Social prescribing helps people to get more control over their healthcare and manage their needs in a way that suits them. It connects them to practical and emotional community support, a range of activities that are typically provided by voluntary and community sector organisations, for example, volunteering, counselling, arts activities, group learning, gardening, befriending, cookery, healthy eating advice and a range of sports.

Women's health

A specialist nurse offers advice on women’s health, the menopause and contraception, including coil and contraceptive implant clinics.

First Contact Practitioner Team

A first contact practitioner (FCP) is an experienced physiotherapist who has the advanced skills necessary to assess, diagnose and recommend appropriate treatment or referral for musculoskeletal (MSK) problems on a patient's first contact with healthcare services e.g., when they visit their GP surgery. We have appointments available 3 days a week and our team can book an appointment with them.
