Your Annual Review – What to Expect on the Day

Thank you for attending your annual review. To help your appointment run smoothly and make sure we have all the information we need to provide the best possible care, there are a few simple steps to complete when you arrive.

1. Check in when you arrive

When you arrive at the surgery, please check in using one of the screens in the foyer.

If you need any help checking in, or if you are unable to use the screen, our Care Navigators at the front desk will be happy to assist you.

2. Use our Health Assessment Machine (HAM)

After checking in, please use our Health Assessment Machine (HAM).

The HAM will record:

• Your height
• Your weight
• Your blood pressure

The results are sent straight to your medical record, so there is no need to write these down or tell the clinician your readings.

If you need any help using the HAM, please let one of our Care Navigators know. We are happy to help.

3. Have your blood tests and other checks

Your Healthcare Assistant (HCA) or Nurse will call you through when they are ready to see you.

During your appointment, they will complete the checks and take any blood samples that are required as part of your annual review.

The tests you need may vary depending on your individual care and medical history.

4. Please bring your urine sample

It is very important that you bring your urine sample with you.

Your urine sample can provide important information about your health and may help us identify problems that you may not otherwise notice.

Please make sure you have your sample ready when you attend your annual review.
