Giffords Surgery Carers Information

Carers Support Hospital Liaison Service

Please see the below poster for more detailed information on this service and what it can do for you as a carer.

There is a telephone number, email and web address should you need more information.

A carer is someone who is looking after a relative or friend who, because of a disability, illness or the effect of old age, cannot manage at home without help. A carer may care for a few hours a week or all day. They may help with personal care, shopping mobility or just keep a regular eye on someone. They may live with the person they care for or visit. They will be unpaid, although they may be receiving benefit.

If you are a carer, please complete a carer’s registration form available from Reception to enable us to add your name to our carers’ register.
